Assistant Public Health Practitioner
Salary: Band 4 - £23,949 - £26,282 per annum (pro-rata for part-time)
Hours: Part time - 22.5 hours per week - Flexible working

This post is in support of the Specialist Public Health Practitioners (Health Visitors) to deliver the Healthy Child Programme of care for 0-5 years and some enhanced programmes to school aged children 5-19 years old.
The Healthy Family Team work in partnership with other agencies to deliver care and interventions to children and families. The team is led by the Specialist Public Health Practitioners, with support from our skill mix colleagues in working with 0-19 year olds within a defined geographic area.
The role include breastfeeding support, completing mandated reviews and supporting the team to meet their Key Performance Indicators The interventions for school aged children include (but are not restricted to) support with emotional health needs. Other duties will include deputising for the SPHP in meetings with Social Care and others, many meetings current being conducted on MS Teams remotely. You will work directly with children and their families to deliver short pieces of work to meet their needs. You will be responsible for onward referrals to specialist services as required.
